You hereby grant MRI Wellness a perpetual, non-exclusive, fully paid, royalty-free, transferable, and sublicensable license to use, reproduce, edit, modify, adapt, publish, distribute, display, and create derivative works from the Materials, in whole or in part. You waive any right to inspect or approve the finished Materials or the manner in which they are used and acknowledge that MRI Wellness will own any derivative works created from the Materials.

You acknowledge that MRI Wellness may rely on this consent and the rights granted herein and agree not to assert any claim, demand, or cause of action against MRI Wellness or its partners arising from the use of the Materials as described.

You represent and warrant that:
(i) you are at least eighteen (18) years of age;
(ii) you have the full right and authority to grant this consent and license;
(iii) the Materials are truthful and accurately reflect your experience;
(iv) you own the Materials or otherwise have the legal right to grant the rights described herein; and
(v) the Materials do not infringe upon or violate the rights of any third party, including privacy, publicity, or intellectual property rights.

By checking this box, you also consent to be contacted by MRI Wellness regarding your submission, and you agree that any additional information you provide in follow-up communications may also be considered part of the Materials subject to this consent.
